Forms are linked to services from the form's Settings tab, not from the service. Each service's Fields & Forms tab shows which forms are already linked, and Policies & Paperwork on a service is for policies and agreements, not intake forms.
Link a form to services
Link a form to every service, or only to the services it is relevant to.
- Open Fields & Forms and select the form on the Forms tab.
- Choose Settings.
- Under Apply to services, choose All services or Specific services.
- For Specific services, pick them under Linked Services. To link a course, pick it under Linked Courses.
- Select Publish Changes, tick the box in Before You Publish, then select Publish Changes.
A course's lessons are not listed under Linked Services; link the course under Linked Courses, which appears once your account has a course template. The form must also be Active to be sent.
To create a form that is already linked to a service, select New Form on the service's Fields & Forms tab.

Check which forms a service uses
- Open Services, select Manage on the service, then Manage Service.
- Choose Fields & Forms.
- Read Intake Forms for This Service.
The list groups forms that apply to All services and forms for This service only. Each row shows the form's field count, whether it goes every visit or once per client, and an Auto-send badge when it is sent after booking. Select a row to open the form, or Manage all forms to open Fields & Forms.
When to collect
There is no separate "Auto-send after booking" switch. On the form's Settings tab, When to collect decides when the form goes out:
- Send after booking: SMS and email after a booking is created for a linked service
- Required before booking: the form appears in self-serve checkout and must be completed before the booking is confirmed. Staff-created bookings still send it afterwards
- Send manually: nothing goes out when a booking is made. Use Send from the client profile
The form must also be Active and linked to the service, or set to all services.
Frequency
Frequency on the form's Settings tab controls how often the form is sent to the same client.
With Every appointment, a new pending submission is created each time a booking is created for a linked service, and an old pending submission does not block a new send. Use it for "Has anything changed since the last visit?" health updates, per-stay feeding instructions and appointment-specific requests.
With Once per client, the form is sent only until that pet owner account has a completed submission. After that, auto-send and manual Send Form are blocked. This is not per pet and not per service: completing the form for one dog blocks it for a sibling on the same owner. Use it for liability waivers, initial health assessments, service agreements and one-time consent forms.
Unlinking a completed form from an appointment does not reset once-per-client, and submissions cannot be deleted. To collect again, change Frequency to Every appointment, or Duplicate the form.
What happens when a booking is created
When a booking is created, by your team or by the customer:
- Petboost identifies the services in the booking.
- For each linked and active form, it checks When to collect and the frequency.
- If the form is Once per client and already completed for that owner, it skips the form.
- If a completed submission of that form from the last 24 hours is not yet attached to a booking, it attaches that submission to the new booking instead of sending again.
- If sending is needed, it sends by SMS and email, unless your team booked silently.
On a staff booking, the form offers Fill for owner or Send to owner. Pending submissions appear on the form's Responses tab.
A service can have several forms linked, and each follows its own frequency. For example, a boarding service might trigger a Health Questionnaire every appointment and a Liability Waiver once per client. A form can also be linked to several services; one form on a booking with several matching services is still sent once.
Unlink from a service or from an appointment
These are different actions.
Unlinking from a service, by removing the service on the form's Settings tab or switching away from All services, stops future auto-sends for that service and does not change existing submissions.
Unlinking from an appointment, with Detach on the appointment or Remove / Change on the Responses viewer, clears which booking the submission sits on. It does not change Pending or Completed, does not trigger a new send, and does not reset once-per-client.
Frequently asked questions
What happens if I unlink a form from a service?
The form stops being sent for new bookings of that service. Existing submissions are not affected. Link it again at any time to resume sending.
Can I link different forms to the same service?
Yes. Several forms can be linked to one service. Each follows its own frequency and When to collect setting.
What if a booking has multiple services?
Petboost checks every service in the booking and sends each linked form that applies. The same form is not sent twice in the same booking.
Do I configure forms from the service settings or the form settings?
From the form's Settings tab. Open the form on Fields & Forms, then choose All services or Specific services. The service's Fields & Forms tab only shows what is already linked.
I detached a completed form from an appointment. Will it send on the next booking?
Not if Frequency is Once per client. Detach only clears the appointment link. Change the frequency or Duplicate the form if you need another send.
