Form Responses

Every intake form has a Responses tab listing each submission, pending or completed, and each client's submissions also sit on the Fields & Forms tab of their profile, where you send, resend, fill in and attach them.

This guide covers viewing, filtering, attaching and managing the submissions your clients send back.

Review a form's responses

  1. Open Fields & Forms and select the form.
  2. Open the Responses tab.
  3. Optional: filter by All, Pending, Completed or Archived, or type in Search client, pet or response....

Above the list, Total, Completed, with its completion rate, Pending and Avg Response Time sum up the form. Group by client gathers each client's submissions together, and the view switches between List, Cards and Grid.

Each row shows:

  • Submitted: the send date for a pending row, or the completion date for a completed one
  • Status: Pending or Completed
  • Sent via: SMS, email, both, Self-service booking when the form was collected in online checkout, or Not sent, with the mobile or email used when Petboost sent it
  • Client, the pet owner, and Pet when a submission is for a pet
  • Appointment, the booking the submission is attached to

Pending and completed

  • Pending means the form has been sent, or created for the client, and not completed yet. Use it to follow up with clients who have not done their paperwork
  • Completed means the client or someone on your team has submitted answers. Use it to review recent submissions or check answers

Read a submission

  1. Open Fields & Forms and select the form.
  2. Open the Responses tab and select a completed submission.

The submission opens with every question and answer, the signer's typed name when the form has a Signature field, a link to each uploaded file when it has a File Upload field, the date and time it was submitted, and the form version used when the form has been edited since. Previous and Next move through the completed submissions.

A client's answers cannot be edited and a submission cannot be deleted. Instead:

  • Correct response saves a new submission from your team with the right answers. The original stays in your records, the client cannot edit the new one, and your name is recorded on it
  • Archive response, after you tick the box to confirm, archives the submission. It stays on the record, and you can still open it from the Archived filter

Attach a submission to an appointment

In the open submission, Attach appointment links an unattached submission to a booking. On an attached one, Change moves it to another of the client's bookings and Remove clears the link. Removing the link does not delete the submission or reset Once per client.

Review a client's forms on their profile

Form submissions live on the client profile's Fields & Forms tab, not the Paperwork tab, which holds agreements and uploaded documents.

  1. Open Customers & Pets and select Manage on the pet.
  2. Open the Fields & Forms tab.
  3. Under Form Submissions, read the client's pending and completed forms.

From Form Submissions you can Send Form, Resend a pending form, Attach a submission to an appointment, Fill on Behalf, or Fill a Pending row. It shows which forms the owner has completed and which are still outstanding.

Forms on the appointment

An appointment's Forms & Responses section shows the submissions linked to that booking. From there you can send, resend, attach and detach. When a booking has several completed forms, they fold into a list you open one at a time.

Detach only clears the link to the appointment. The status stays the same, nothing new is sent, and Once per client stays satisfied when the form was already completed.

Attach rules

  • A submission attaches only to an appointment for the same client
  • You can move a submission from one of that client's appointments to another
  • You cannot attach a submission to a different owner's booking
  • If a client completes a form that is not linked to a booking and then books within 24 hours, Petboost attaches that submission to the new booking instead of sending the form again
  • A submission links itself on completion only when exactly one matching upcoming booking exists. When more than one matches, it stays unlinked until you attach it

Follow up a pending form

  1. Open Fields & Forms and select the form.
  2. Open the Responses tab and filter by Pending.
  3. Check Sent via to see whether the form went by SMS, email, or was not sent.
  4. Select Resend on the pending submission, from the client's Fields & Forms tab or from the appointment, or contact the client yourself.

Resend reminds the client about the same pending submission. Send Form creates a new one, and is refused when the form is Once per client and already completed.

Fill a form on behalf

Pending means the form has been sent and the client has not completed it. Your team can complete that row, or start a new submission, from the client's Fields & Forms tab.

  1. Open Customers & Pets and select Manage on the pet.
  2. Open the Fields & Forms tab.
  3. Select Fill on Behalf for a new submission, or Fill on a Pending row.
  4. In Fill form on behalf, choose the Form if you started a new submission.
  5. Optional: choose the pet under For pet (optional), or leave No specific pet.
  6. Optional: switch on Notify client.
  7. Enter the answers, then select Complete Form.

The submission is attributed to you, and it counts as Completed for Once per client. See Intake Forms Guide.

Frequently asked questions

Can I export form responses?

Answers linked to custom fields fill in those fields on the profile. For reviewing many at once, use the Responses tab, which lists every submission with filters and a Grid view.

What happens to responses if I edit the form?

Existing submissions keep their original answers, based on the form as it was when they were submitted. New sends use the updated form.

Can a client resubmit a form?

With Every appointment, each booking creates a new submission. With Once per client, any completed submission for that owner account blocks another send, and detaching it does not change that. To fix an answer yourself, use Correct response.

Can I delete a submission?

No. Submissions cannot be deleted; they stay as a permanent record on the client's profile. You can archive one with Archive response, and it stays on the record.

How long are submissions stored?

Submissions are kept permanently. They stay on the client's Fields & Forms tab and on the form's Responses tab for as long as your Petboost account is active.

No. Remove and Detach only clear which booking the submission is attached to, and a completed submission stays completed. For Once per client, change the frequency or Duplicate the form if you need to send it again.
